44 yr old lady ,not a k/c/o DM/HT/Dyslipidemia c/o dyspnoea on exertion for the past 3 yrs along with 10% weight gain. On examination bilateral pitting pedal edema present more so on the right leg (photo attached) vitals are stable and all the systemic examinations normal. Also attaching the photo of her dry,coarse skin,mostly due to hypothyroidism P.s:Can pedal edema be severe enough to cause medial deviation of the toes??(photo attached) If not what are the reasons for it??

A middle aged obese female ( BMI - 30) a k/c/o Hypothyroidism presented to ED with complains of Pain Right Hypochondrium, weight gain and Anorexia since 1 month....On examination, she was found to have Hepatomegaly (4 inches below the right costal margin)....USG and CECT W/A was done....Comment on the treatment approach to this patient...

23 yrs male student by profession came with the complaints of hearing heart sounds while sleeping on left side and SOB classifying grade 2 of NYHA... Vitals:- Bp-140/100 in both arms fluctuating on repeating..R.R-18 per minute(At rest) ..heart beats around 100-105 per minute..non diabetic.. Patient observed this changes since he left the gym workout in the lockdown..he gained nearly 12 kgs in the lockdown with out any work,...plz interpret the reports below showing mildly elevated haemoglobin and pcv, lymphocytes and basophills..RFT is normal...Is it diagnosed as polychythemia vera?? What tests should be done and how to approach this case further.. And treatment plz

Dear friends and fellow colleagues, This is an X Ray Foot, AP & Lateral views, Patient is suffering from T2DM HTN Dyslipidemia Hypothyroidism. Obesity Grade 2. All parameters are normal. C/O Ankle pain from last One year or so. O/E Mild putting Oedema around Ankle. No signs of Infection. ESR normal. X Ray Findings & Treatment please.

60/F presented with h/o developed difficulty in breathing a/w chest discomfort present while on travel consulted elsewhere and admitted for further management.h/o giddiness + k/c/o T2DM, SHTN, Hypothyroidism on rx o/e conscious, oriented, dyspneic HR: 92/min BP:140/90mmhg. SPO2:94% investigation and CAG done.. Diagnosis and treatment?? is possible trop I elevated with normal coronaries??
