Case of jaundice without fever

C/O 24 yr , male with C/C of High colored urine since 3 days , yellow discoloration of eye and sublingual mucosa since 2 days , fatigue and mild discomfort ( pain) at hypogastric and epigastric region . No H/O Pale colored stool , Pruritus , Fever , burning micturition No past H/O of taking any drug , alcohol . He is a vegetarian with adequate bowel and bladder . He was on uncooked food and raw vegetables since last 10-15 days ( Quarantine period ) LFT and routine urine microscopy reports and there . what will be the possible diagnosis and management?

It appears from the reports a case of viral hepatitis as all liver enzymes are raised . Sr alkaline phosphatase raised may be due to cholestatic oedema . Blood test for Hepatitis B AE USG of upper abdomen to see liver texture and gall bladder stone Absolute bed rest Bland diet by mouth if there is no vomiting or if pt can tolerate If there is vomiting nutritional support by IV infusion Inj dextrose 10% iv two bottles daily for 3-5 days to give rest to liver Inj multivitamin infusion one bottle daily To repeat LFT after 7days if bilrubin does not show lowering tendency then Steroid Tab Prednisolone may be given

The odd point of viral hepatitis is absent of fever and prodromal symptoms. Still send viral profiles..but pls consider autoimmune hepatitis also..Anti LKM. Anti sm antibody. Recently one of my patients was found to have Wilson disease with similar history.
