SGLT2 Inhibitors like Empagliflozin, Dapagliflozin etc can not be recommended when the eGFR is below 45 mL/ min/ 1.73 m2. But According to the CREDENCE Trial , Canagliflozin can be recommended up to eGFR 30 mL/ min/ 1.73 m2. Coming to the Metformin , it is contraindicated in patients with an eGFR less than 30 mL / min / 1.73 m2. The dose of Metformin should be reduced, in patients who are already on Metformin ,when eGFR falls below 45 mL/ min/ 1.73 m2. and monitor eGFR in every visit and Stop Metformin when eGFR is falling down. Starting treatment with Metformin in patients with an eGFR between 30 and 45mL / min / 1.73 m2 is not recommended .
Grading of eGfr Above 60 is N From 45 to60 is gr A From 30 to 45 grBa 15 to 30 Bb Below 15 is c or critical Latest studies have suggested SGLT2i can be used upto gr c
egfr bw 30 to 45 is satisfactory ,,below that always take into consideration the drugs risk benefit ratio
No dose adjustment is needed in patients with an eGFR greater than or equal to 45 mL/min/1.73 m2.
Stop metformin If eGFR is less then 30 to 45
