Fissure In Ano

A 25year old male patient, C/o Pain in the anal region since 3days, and had a pimple like symptoms, pain during defecation, no h/o fever. O/e there was redness palpable tenderness present ,and as shown in image lesion is 2 to 3cms above the anal canal.

A midline ulcerative lesion 2- 3 cms above the anal opening .External anal sphincter also looks in spasm . Looks like acute fissure in ano with secondary infection and an infective intertrigo associated lesion as mentioned. Treatment is . 1.Sitz bath 2.Augmentin 625 mg B.D for 1 week . 3.Tab Aceclofenac 100 mg + serratiopeptodase B.D for 1 week 4.Topocal Metronidazole + mupirocin ointment. 5.Laxatives - softovac 2 tsf bed time

Perianal abscess R/O Inflammatory bowel disease eg Crohn's disease or Ulcerative colitis Sitz bath in warm water medicated with betadine for 10 minutes twice daily I/D to be given Antibiotics Antiinflammatory Analgesic Purgatives Local Pilex ointment p/r 4-5 times daily If does not heal refer to Surgeon Also see if there is Anal fissure

It appears to be anorectal fistula Pimple like swelling is external opening of tunnel appears as blister ie about to burst He needs fistulactomy
Will botox be helpful?? In treating the symptoms faster???
