What can be done?

57 yr old female, complaining of pain & tenderness in B/L leg and ankle, with swelling. H/o B/L TKR two years back with k/c/o DM, Hypothyroidism, & multiple joint pain. Can u suggest the differential diagnosis and treatment accordingly?

Rule out inflammatory arthritis CBC esr RA factor Uric acid Crp
Stasis dermatitis,may require a course of antibiotics Compression stockings Diuretics is in order if significant non tender edema As already suggested Doppler,urine routine and Renal function,ser.albumin, In view of arthritis,viral arthritis, hyperuricemia, ANA, ANCA, RA factor, Will need XRays to rule out chondrocalcinosis, If upper limb arthritis, wrist and hand consider hemachromatosis too

Lady had got multiple health problem with bil total knee replacement. Take care of diabetes and hypothyroidism Go for kidney function test to see the kidney status is responsible for swelling or not Go for Doppler study to asses the perepheral vsacular disease like venous stasis causing oedema Weight reduction Rule out pt is ambulatory and on regular exerscises or not

Lady has multiple comorbidies DM2T hypothyroidism and polyartheralgia H/o TKR B/L Pictures suggest dependent swelling needs to workout freshly for associated conditions Adv colour doppler study And orthopedics opinion to review TKR xray knees to see prosthesis is in situ and not loosened Meanwhile symptomatic treatment
