It is one sided lower abdominal pain associated with ovulation. It is dull and cramp like, Sudden and sharp , Some times associated with mild vaginal bleeding. Site of pain may change depending on side of ovulation. Cause is exactly not known. Probable cause could be 1) Just before egg is released with ovulation, the follicle growth streches the surface of the ovary causing pain. 2) Blood or fluid released from the ruptured follicle irritates the lining of abdomen i.e.peritoneum , causing pain. This Mittelschmerz pain doesn't require any treatment. If required ,give NSAIDS to releive pain.

Mittelmerz phenomenon is pain during or after ovulation . Following ovulation fallopian tube contracts to facilitate sperm for fertilisation ... Blood and fluid from ruptured follicle enters the abdominal cavity and pelvis causing irritation resulting in pain.

pain usually goes away within 24 hours, so treatment is usually not needed. Over-the-counter pain medicines—such as ibuprofen—are usually effective in relieving mittelschmerz. Women with very painful ovulation might find relief by taking birth control pills, which prevent ovulation.
