A 35 year old man presented to ED with complaints of cough, low blood pressure, swelling on lower limbs since last three month. H/O fever Plasmodium falciparum (duration one month) two month back resolved after taking antimalarial medicine. His physical examination showed a temperature of 98.6 °F, blood pressure of 90/60mmHg, heart rate of 78 beats/min, RR 20 breaths/min, and an oxygen saturation of 90% on room air. No respiratory distress. His conjunctivae showed no evidence of pallor and sclerae were anicteric. His jugular venous pressure was normal. His heart rhythm was regular, without a third or fourth heart sound. His abdomen was soft and non-distended. lower extremity edema appreciated with asymmetric leg swelling, or calf tenderness, or clubbing.
