Large Acute Cerebellar Infarction with Hemorrhagic Transformation.

New Case. 50 yr ,F, known Diabetic and hypertensive,on irregular follow up presented with acute onset of severe vertigo with vomiting and unsteadiness. Severe subjective vertigo ,vomited several times and then developed unsteadiness Evaluated at loc hospital, CT brain was done reported as normal.With in a few hours ,sensorium deteriorated and hence intubated and mechanically ventilated and then transferred and arrived 16 hrs after the onset of symptoms. On exam deeply sedated ,Bp170/ 110mmhg pupils bilaterally 2mm,poorly reacting.Absent DTRS with 0 plantars. What abnormality in the MRI brain?.

Discussion - What abnormality in theMRI brain. ●Large Acute infarction with hemorrhagic transformation with in the RT Cerebellum ( involving the postero superior,antero inferior , postero inferior quadrants), extending to the vermis and postero superior part of left cerebellum. ●Moderate surrounding edema with effacement of cerebellar folia. patient still l on ventilator with active supportive managemwnt.
