The history can differentiate subcortical from cortical disease by focusing on 4 major areas. 1.The presence of specific cortical deficits including language function& visual- spacial problem. 2.Pattern of motor and sensory deficits. 3.Type of sensory deficits 4. Presence of visual field deficits. Note also that seizures are almost always cortical in origin. Elaboration of each one is difficult in the space provided. History of impaired cognitive function and seizures, presence of apraxia or aphasia indicate cortical involvement
