25yrs obese ,PCOS,hypothyroidism female with these following findings. Your opinion please.

Acanthosis nigricans / Contact dermatitis from necklace. Ask for itchiness, Take personal or family history of diabetes. Advice : 1. Levothyroxine for hypothyroidism. 2. Birth control pill for PCOS. 3. Reduce weight with diet and exercise. 4. Avoid wearing necklace. 5. Advice Fasting blood sugar, Fasting Insulin, Free testosterone, DHEAS, 17 alpha hydroxy progesterone. 6. If contact dermatitis then apply topical potent steroid.

Acanthosis nigricans along with contact dermatitis. AS the pt is obese and have PCOS, likely to have some level of insulin resistance. Check for blood sugar levels, TSH, Treatment includes local application of topical steroids, to continue thyroxine, control of blood sugar levels if it is high, and of course increase physical activity to reduce weight. Over treatment and over investigation not required.

Acanthosis nigricans. Probably applied steroids locally to reduce the hyperpigmentation.
