Considering cxray, CT thorax picture showing densed opacity without tree in bud appearance, Elderly age , emphysema, findings favours malignancy than TB. Clinical history, like occupation, smoking and clubbing if available, would be great helpful.
Rt mid zone opacity inhomogenous lesion Scarring in rt apical area Discreet infiltrates in rt mid lower zone Most likely tubercular To r/o malignancy biopsy is advised

Homogenous appearance in Rt mid zone And discreet infiltration in Rt mid lower zone. Indicate towards possibility of KOCH'S CHEST. MALIGNANCY SHOULD BE RULED OUT.

SUGGESTIVE. Of .. CARCINOMA MALIGNANT CHANGES... ADVISABLE... BIOPSY ALL. ROUTINE. INVESTIGATION RULE. OUT. ..P. TB
Collapse consolidation right middle lobe D/D tuberculosis Malignancy Ad CBNAAT Sputum exam HRCT thorax
This is look like Carcinogenic with Mets, Whole body PET Scan needed. But nothing can be done.
Suspicious of Malignancy Adv: biopsy
? TB ? MALIGNANCY Suggest sputum for AFB and CBNATT, bronchoscopy , BAL , for AFB AND CBNATT, and cytology Biopsy of lesion and lymphnodes
