X-ray suggest cardiomegaly with right hilar enlargement and right paratracheal whiteness! Fever off and on suggests immune deficiency condition---- so exclude carcinoma and tuberculosis! Breathless with pedal edema can be chronic chest infection or due to heart --- go for ECG and echocardiography and ABG !

Mahy be congestive H. Failure/pulmonary oedema. Xray shows pulmocardiomegaly.Do ECG, SPUTUM for AFB.Rule out carcinoma/TB should be rruled out .Take physician reference.
