patient suffering from this lesion last 15yrs .age 55yrs,male. non diabetic.treatment done before 10years ago.at present no such medication last 5yrs. please Rx/DX.

On perusal of foot it looks to be a c/ o leukoderma /vitiligo bcz of that vary poor circulation in affected area resulting cracks and ulceration since it seems never advised for off loading the wt on affected foot hence a nonhealing trophic ulcer .tvnc should be r/o by tissue biopsy Rx proper debridement and dressings with normal saline should give granulation rest is necessary give broadspectrum antibiotics send c&s Rest of treatment as per biopsy and culture reports

Chronic non healing foot ulcer ? Hansen's ulcer with secondary infection due to poor hygiene ? marjolin ulcer As it's mentioned, pt is non diabetic, do his recent sugar profile with hba1c.. can't diagnose as diabetic foot ulcer unless he's lebelled as diabetic Is he a SMOKER?? Cholesterol profile? ? vascular stasis ulcer ? pyoderma gangrenosum

X-ray Biopsy for histopathology See for diabetes, reynauds,neurosensory impairment, bone infection Once they are done consider for Wide excision and skin cover If malignant consider for amputation.

Chronic non healing gangrenous ulcer. Ad- Colour doppler LL to rule out thromboembolic disorder, Skin biopsy. Start oral anticoagulants,, broad spectrum antibiotics
