Somogyi phenomenon is high blood sugars in the early morning . This is because of excessive insulin dose at bedtime or missing late night snack which results in low blood sugars in the early morning , and increased counter regulatory hormones like cortisol, catecholamines and growth hormone etc resulting in high blood sugars in the morning. This should be differentiated from Dawn phenomenon by testing early morning blood sugars at 2 to 3 AM . If blood sugars are low at 2 to 3 AM, it is Somogyi phenomenon. If blood sugars are normal or high at 2 to 3 AM ,it is Dawn phenomenon. Treatment of Somogyi phenomenon is Reduce the night dose of insulin and giving late night snack before sleep.

Sustained raise of fasting plasma glucose ,in an Insulin regime patients ,one should suspect ,insulin excess at his bed time .So reduce night insulin administration.Somogyi effect to be monitored in all who are on night plain insulin.

It's seen in Diabetes patient mainly insulin dependents. Occur- at morning. Features- High blood sugar levels in the morning. Trigger factor- Cortisol hormone, epinephrine, growth hormone, glucagon. Events: It does not occur regularly. It happens when patient takes insulin at bedtime, sugar falls gradually at the very low level( hypoglycemia occur), then there was stress in the body and the triggering hormone secrete and elevate the sugar at very high level.

Diabetic patients sometimes gets hypoglycemia in night time As a homeostatic mechanism, busy tries to correct hypoglycemia. This leads to rebound hyperglycaemia in morning. This is called as Somogyi phenomenon Therefore, morning blood sugar is checked, and if there is hyperglycaemia, then one should check for early morning blood sugar before increasing dose of diabetic medications

Insulin taken before going to bed and it causes high blood sugar level during early hour of morning due to rebound effect.
