9month baby with watery stools 12-15times a day, should one give antibiotic ,how long should one wait?

As per case History and available information 1. baby is of 9 month 2. Watery stools 12- 15 episode /day. 3. CBC - Hb 6.4, Microcytic hypochromic anaemia with normal leucocytes count and mild lymphocytosis. 1. Child is required to be evaluated for amount, consistency, blood in stool and relation of stool to feeding. 2. Type of feeding, i.e breast, or top if top than is it cow or formula. And the method of feeding 3. One should calculate the nutritional gap. Although this is a typical case of 1. Top feeding especially with bottle with that led to frequent gi infections and improper dilution further increasing the nutritional gap. 2. Lymphocytosis with this much of Anemia with normal count giving another indication that perhaps child is on Cow milk and that is causing cow milk allergy that may also produces similar course of illness. How to deal with it. A. Stop Bottle feeding immediately. B. Stop cow milk if baby is on that C. Check for stool for microscopy, occult blood, stool for reducing substances. D. Shift baby of planned diet as per persistent diarrhoea protocol. E. Educate family on exclusive breastfeeding and complementary feeding based on family pot feeding. Best Regards Dr Manish Kumar

No anti biotics to start with diarrhoea is caused by Rota virus so give prebiotics& o r s hydration has to be maintaned send stool exam if blood or pus cells more then 10/ hpf then we can start antibiotics Stool for reducing substances to rule out lactose Intolarance wiat for 4-5 days

Practically. Inj mikacin 125 mg +inj Gentamycin 40 mg bd. Probiotic and rehydration. Change in feeding , councelling to family required. If baby is not taking ORS in adequate quantity change to iv infusion.for 5 days.

Most acute diarrhoea are viral, no need for antibiotics, test stool if pus cells more than 10,or blood in stool give antibiotics. Test for lactose intolerance by Benedict test.If 3+try lactose free foods

Clinically if mucoid and associated with smell,greenish coloured u can try an antibiotic course with syp cefixime.give more of oral fluids ,ORS and probiotics

It's viral diarrhea with iron deficiency anemia . Give blood transfusion .for quicker recovery of patient . Zinc probiotics n ors or i'v fluids . Good counselling of parents .
