Pleural effusion with pericardial effusion

A 21 year/ female pt came into OPD with c/o pain in left hypochondrium , increase while deep breathing and coughing also have h/o dry cough, low grade fever and generalised weakness. Pt taken treatment for upto 3 days with her local doctor but there is no improvement. Pt CBC report shows Hb-9.0, rest all pathological reports are normal and advise her after S/E for CXR and USG-abdo/pelvis report. Here i attach the reports of CXR and USG-abdo/pelvis. It is s/o left sided pleural effusion and pericardial effusion. So what is the treatment modalities given to this pt? Is pleural tapping required? Kindly suggest treatment approach in this case.

Boot shape cardiac shadow suggestive of significant pericardial effusion Homogeneous opacity lt lower zone too consistent with pleural effusion Yes pleural tapping should be done for diagnostic and therapeutic purposes Looks to be a c/o tubercular pericarditis with pleural effusion After investigations pt may be put on ATT

Possibly tubercular pleural and Pericardial effusion. Adv diagnostic pleurocentesis and medical management accordingly.

Lt sided pleural effusion do pleural tapping, send pleural fluid for AFB ne and cyto/bio/GS.start ATT if it comes tubercular
