A 22 year old female has swelling and pain of 1 st MTP joint (rt), right knee and one MCP joint (rt) and generalized weakness since 4+ months Pt has hypothyroidism Consulted orthosurgeon gave the following treatment but no change Please give your advice diagnosis and treatment Chief Complaints Difficulty in getting up Pain in the joints Tenderness Redness Swelling Weakness History Hypothyroidism No history of fever Investigations Reports attached

Most likely gouty arthritis with hypothyroidism. Advise-serum uric acid . Thyroid profile. FNAC of involved joint may crystal of uric acid tophae crystals. Rx-Tab febustat 80mg 1od. Tab piroxicalm 20mg 1od. Tab becoplus 1od×1month.
