A 22 yr old male c/o of diffue abdomen pain more on right side on nd off since 6 month... Pain is more in last 2 weeks... O/E abd is rigid... Vitals stable... Ct abdomen report attached... Kindly suggest the diagnosis nd line of management..
Repeated episodes of pain abdomen in a 22 years old male with CT showing peripancreatic fluid collection with raised amylase and lipase is s/o recurrent acute pancreatitis. Conservative management with IVF and analgesics. Early enteral feeding to be started. After resolution of the acute episode, work up the patient for cause of RAP with serum calcium, complete lipid profile and MRCP. If above negative plan, genetic workup.
Most likely a case of acute Pancreatitis. Conservative treatment is indicated 1.NPO 2.RT suction 3.IV fluids 4.Parenteral antibiotics Inj Piperacillin 4 gm + tazobactum 500 mg × 6 hourly 5.Intensive monitoring of urine output, s calcium, electrolytes 6.Option for surgery to be kept in mind

Pain abdomen for 22 years old man and off and on pain abomen with high rise of pancreatic enzymes souggestive of chronis recurrent pancreatitis. This time pt is getting more pain abdomen for last 2 wks is highly suggestive of ACUTE ON CHRONIC PANCREATITIS. P At this stage pt to be treated conservatively NPM IV drip to maintain nutrtion Inj Piperacillin 4 gm + tazobactum 500 iv 6 hrly Multivitamin infusion Keep intense watch for the vitals with urine output Lipid profile and serum calcium CT scan As the is having for as t the age of 22 years old may be consider for operation considering all merits and demerits.
