A 23 year old woman came in my clinic, complaining with pain abdomen, nausea with vomiting, fever with general weakness. O/E B/P 100/60mmHg Pulse 88bpm Temp. 100°F R/R 18bpm Gestational age is 14 weeks 5 days Suggest best treatment....

Complete urine exam incase infection culture sesitivity meanwhile symptomatic treatment usG for foetal wellbeing or signs of chorio amionitis

nausea nd vomiting may be pregnancy induced tab doxinate 1bid tab ondem in addition if needed pain abdomen if in pelvic region with fever may be due to UTI as it is common during pregnancy she needs cephalexin or amoxycillin and paracetamol

- As such routine ANC - give IV fluid+ inj. Ondecetron+Dexona+ pentoprazole - symptomatic tab. PCM sis - dry fruits , fruits, frequent liquids, ice cream, juice

Complete blood count ,GBP for malaria,widal test Start tab paracetamol 650mg tds for 3days Tab aciloc 150mg OD ,Tab ondansetron 1 tds Tab folic acid 1OD Oral rehydration solution Start iron tablets after 1 week once gastritis subsides

Basic investigations need here to rullout cause of fever. Abdominal pain may be secondary to gastritis.treat pt.accordingly

Maqusud bhai , did you have come to diagnosis ? Are you are not come to final diagnosis since one month even tho pt. Is with you till today ? If so we admit ,we are very very very very very very poor in subject . Please expose yourself ?

NEED TO HAVE ANC PROFILE,S.WIDAL,SGPT REPORTS DONE GIVE HER SYMPTOMATIC TREATMENT ORAL/INJECTABLE (ONDENSETRON, PANTOPRAZOLE) AS PER RELIEF TILL REPORTS AS NAUSEA/VOMITTING SUBSIDES IMPROVE HYDRATION WHICH RELIEVES WEAKNESS IF HYPER EMESIS IS THE CAUSE IT SHOULD BE FROM EARLIER DAYS OF PREGNANCY & NOT AFTER 14 WKS OF PREGNANCY, SO ADVICE ACCORDINGLY

Fever is not significant .follow .nausea vomiting since when ? If since early gest then no issue but if recent then advise pantoprazole infusion if admitted might help .go for a upper abd scan if vomitin and pain persist.
