Rheumatic heart disease with severe Mitral stenosis

A 25 years old female patient having DOE III and intermittent fever she is known case of Rheumatic heart disease but was on medication. CECT Thorax and ECHO attached for discussion.

Hrct suggestive of huge cardiomegaly and enlarged Lt atrium Rt pulmonary artery is dialated and prominent Basal congestion is seen b/l 2decho suggest ms/nr consequences of RHD Pt is in failure As clinical status improves with MX by diuretics and lowering of PAH Adv for valve replacement
