A 60 year old female with history of severe right knee area pain, around the lateral aspect (not on joint line), tenderness along biceps femoris tendon. Pain radiates to lateral aspect of right leg and heel, and also she feels tightness and radiating pain in posterior aspect of thigh radiating till back of buttocks. Her uric acid was high, she complained stiffness of left knee. X ray knee is suggestive of minor osteoarthritic changes, doesn't correlate with pain severity. ANA, ANTI CCP, RF, HLA B27, negative, Lft, lft, cbc normal except for mild anemia and MCV 104 Etoricoxib 90 mg slightly relived pain but mostly Gabapentin and piroxicam led to decreased pain. Now from past 3 days she is having vesicular painful reddish based lesion along the same lateral distribution of leg which is also on here sole and she feels pain and prickly sensation on walking., No fever. There is no comorbidity. Not diabetic or hypertensive. Have been on febuxostat since 6 months for increased uric acid. I am attaching pictures of lesion here. Kindly help.
