A 35/m was traveling to Delhi,was brought to emergency by police with c/o GTCS like episodes with severe breathlessness.on routine examination his creat was 21,spo2 80% on room air,GSC was e2v2m1., BP 220/110.his imaging are attached here.i have made a diagnosis of Newly diagnosed CKD stage 5 with egfr 4 with syst htn with A/s cause uremic enchephalopathy with hypertensive enchephalopathy with dysponea cause fluid over load.3 MHD has been done still no improvement.creatvhas come down to 15 only.kindly give ur expert opinion regarding management of this patient,as he is destitude.

Pt may have this condition is due to hypertension It looks more on hypertensive encaphalopathy. Ureamic condition has aggravated the symptoms. S creatinin upto 21 level without any complain is dout ful. Pt should b seen by nephrologist and neuorologist.
