A thrombosed external haemorrhoids. Needs conservative treatment Sitz bath Tab Aceclofenac + serratiopeptidase B.D for 1 week Tab Ornidazole 500 mg B.D for 5 days Topical lignocaine + metronidazole + sucralfate cream . I'd conservative treatment fails- evacuation of clot after incision and drainage

Sitz bath once after defaecation and once in the evening Avoid constipation Laxative to keep stool soft Xylocaine 2% gel to apply inside anal canal 10/15 minutes before passing stool and after taking sitz bath once again Antibiotic and Analgesic may be given

As per my opinion it's a case of 2nd to 3rd degree Intro-external haemorrhoids @ 3 O' clock position with Thrombosis; Spasmodic anus. Please do DRE first to check proctitis ? If yes then suggest sitz bath with Proctobath liq. 3-4 times per day then do Proctoscopy to evaluation of haemorrhoids. Closed haemorrhoidectomy with Lord's procedure under SA is choice of Sx.
