Skin Tuberculosis with cold abscess

A 48 years male with history 3- 4 satellite lesions over left infra - mammary region A biosy from the lesions,was indicative of tuberculosis. ATT was started - category 1 drugs After one month of treatment all satellite lesions were,coalesced to form a cold abscess at the site of lowest lesion. Aspiration of the cold abscess was done with a wide bore,needle and 60 cc of abscess was aspirated. The photographs are before aspiration of the abscess

Yes Doctor. Please continue Antitubercular drugs. Still rule out Staph,Strep Infection as well since same kind of lesion is seen in both Yeah ATT covers many bacteria Good case... Thank you Doctor

As it was diagnosed by biopsy and proved to be a case tuberculosis and ATT was given afterward there was an abscess formation and aspiration done with 60 cc of was aspirated but clinically seems to secondarily infected as there is redness of the surrounding skin. So it is better to go for incision and drainage of the abscess cavity with antigravity incision and may closed after complete drainage with a drain in situ. Pus culuture and sensitivity and smear for AFB ATT to be continued with antibiotic as per report and regular dressing after removal of drain after 48 hrs and hope it will take 8- 12 weeks to heal up.

For incision and drainage the abscess is cut open with a scalpel (blade) to release the infected fluid. A drain may be inserted into the wound to help the infected fluid drain or may be left open so that the infected fluid drains naturally. Aspirated pus culture for AFB and other bactarial culture and senstivity must be done.to exclude any secondary bactarial infection. Continue ATT tt.add on antibiotic after awating reports avaialable.

Cold abscess over chest is unusual site ? Exclude other bacterial infection also by C/S test of pus drained with gram's staining & microscopic cellular study. Also blood sugar & chest X-ray & AFB staining. Also trace primary site of tuberculous infection including spines. Hope there shud be no recurrence of cold abscess in future. ATT is responding nicely,this is also a diagnostic point. Cold means cold & not warm & no inflammation. Is there was absence of classical signs of Inflammation during primary manual exam of abscess site. ? ? Just interested to know for our knowledge. Though it may be itself isolated primary site without involving any other part or organ of body. ??
