A 5 yr old girl presented with severe headache and vomiting for the past 4 hours. Her mother denies any history of trauma to the head. She seems to have limp in her gait recently, mainly supporting herself with her right leg. no delay in milestones. Has anemia since age 3 years. What is your opinion? What is diagnosis for marked area?

Sub aute infarction-RT MCA Posterior division.Another small well ciircumscribed appeared as small lacune lt lateral to3rd ventricle.suggests further investigations including vasculitis& Moya Moya disease . Kindly give feedback. Nice case.

Hypodensity seen in parietal region Limp is because of involvement of motor cortex Most likely neoplastic lesion with perilesional edema Infarct is also a differential but onset is not acute as per the history of limp

What blood investigations report says,Rule out the cause of anaemia.The stroke may be anaemia induced.

Dr Vedwan. What is the Hb value of the pt.Is there any sickling?.Is she Thalassemic?.5 yr old girl with one major stroke ,another lacune lt side needs further investigations.Angiogram including root of aorta is absolutely mandatory. Every case teaches as

Most likely infarct in rt inferior div of MCA Get mri brain with mr angio of brain n neck vessel Later other investigation for stroke in young like cardiac evaluation thrombophilia profile vasculitic marker

It will be worth doing MRI of spinal cord to find out if there an old infarct. Present MRI looks like cerebral infarction in parietal region , it's important to find out the cause for anaemia ,& whether that's the cause for thrombosis.,

Cerebral infarct MCA territory..anemia is one of the risk factor,others are sickling moya moya disorder of coagulation..trauma etc...
