A 50 y/o/m came with a c/o exertional shortness of breath and cardiac murmur

Chief complaint- A 50 y/o/m came with a c/o exertional shortness of breath and had a cardiac murmur for last 2 months History- hypertension (on medication), non-smoker, non-alcoholic Vitals- Pulse- 90/min, BP- 130/80 mm Hg, Temp- normal, weight-90 kgs Examination-No signs of heart failure, soft blowing pansystolic murmur was audible over the mitral area, basilar rales were auscultated over the lower lung fields. Investigation- Chest x-ray done. ECG is advised Please guide about the diagnosis and management.

Case of CCF from DCM

CXR STUDY .. CARDIOMEGALY .. PULMONARY CONUS .. NEED'S .. CARDIAC PROFILES ..

Cardiac shadow is enlarged with left ventricular preponderance Double right cardiac contour. Left atrial appandage area and pulmonary conus bulged. Aortic knuckle hypoplastic. Both lungfields are clear. Mitral valve disease with PAH.

Cardiomegaly with prominent pulmonary conus.... Bibasilar rales on examination s/o HF.... so it may b case of DCM with CHF/ pericardial effusion... 2D ECHO to be done urgent...
