fungal infection in foot

A 50 year female patient presented with red, itchy skin lesion in left foot for 3-5 month. These started first from toes and web . Now these leision grows towards upward. History of skin peel and severe itching. First she applied prescribed antifungal ointment. Then now she is using lozivate mf ointment (once at night time ). She gave history of recurrence. What is the diagnosis? Provide management.
