A 55 yrs old male pt have these leisons on Penis no pus mild itching pt is diabetic taking medicines These are since 15 days He apply many otc ointments but no relief Kindly suggest dx and rx ??

Dx :- Penile Candidiasis. Curd white lesions predominantly over glans and scanty over corona and prepucal skin. Advice : - 1 ) Swabs from lesions for Gram's stain, KOH Preparation and fungal culture SOS. 2 ) CBC, FPG, PPPG HbA1C, LFT AND RFT and Anti HIV 1&2 Ab test. Rx :- 1 ) Good glycemic control is must. 2 ) Onabet 2 % cream - Locally BD / TDS. 3 ) Tab Terbinafine 250 / 500 mg - 1 tab BDPC / ODPC HS for 2 - 4 weeks. 4 ) Antihistamine of choices 5 ) Multivitamins tab / cap - 1 tab /cap per day. 6 ) Maintenance of good personal hygiene.

Most common presentation in dm, balanitis by candida albicans, tt good glycemic control with oral flucunazole 200 mg twin a wk, antihistamine, if active family life tt partner with oral antifungal & local application of antifungal. ...

BALANITIS ..cause? ..Fungal .MostlyCandidiasis ,Viral (HPV) 1.Control blood sugar with diet n medicines. 2.Ask about history of exposure to r/O STD esp.syphilis n HIV. 3.Orally Tab.Cifixime 200mg +Azithromycin250mg BID for 7 days. 4.Tab.Fluconazole 200mg twice a week for 1 month. 5.Locally Nadifloxacin+Miconazole +Neomycin cream.OR Fusidic acid+Beclomethasone +Clotrimazole. 6.Wash affected area with soapy water.Maintain Good hygiene. 7.Avoid Physical sex relations till complete cure.
