a 6yrs fch came in OPD with c/o pain and swelling in lt leg snc 3 days o/e tenderness +,local temperature hard swelling ,redness, regional lymphadenopathy peripheral pulsations present, no h/o,trauma insect bites I request to all respected doctors please suggest your opinion and management

It's a spreading cellulitis in subcutaneous plane from the external wound with impending abscess formation. Start with antistaphylococcal antibiotics. I/D when abscess formation is complte and localised
It is likely to be cellulitis One has to watch for fasciitis Clindamycin should be included in regimen

40 year old male, no h/o comorbidities, presented with pain and swelling of the right foot. No h/o trauma, fall. On examination has local rise of temperature, pitting pedal oedema and tenderness. Ankle joint movements are normal. Dx and Rx please.

A m/72 was suffering from pain & bilateral pedal edema from last few weeks. The Pt had a h/o bleeding piles for long 3 months. O/e- hb%-6.8, ESR- 65, CRP +, ASO TITRE - VE. Refereed for transfusion of 2 unit whole blood with medication. After 5 days Hb raised to 9.2, ESR 15, CRP + with rt sided pedal edema ++. Opinions solicitated.

New Case 81 yr , M, Diabetic 6 yrs, on single oral med , well under control , developed back pain 3 months, progressive difficulty in walking 3 weeks . Denied having any sensory / bowel/ bladder symptoms. No fever / loos of weight Or appetite.No symptons suggestive of upper limbs / bulbar involvement/ head ache / vertigo. Examination : general - normal.Intact peripheral pulses. Higher function , cranial nerves & upper limbs - normal. positive beevors sign.Lower limb power gr 3/ 5 with spsticity , hyper-reflexes with extensor plantars . Intact sensations. possible diagnosis ?
