a 6yrs fch came in OPD with c/o pain and swelling in lt leg snc 3 days o/e tenderness +,local temperature hard swelling ,redness, regional lymphadenopathy peripheral pulsations present, no h/o,trauma insect bites I request to all respected doctors please suggest your opinion and management

Dear Dr. Rajendrakumar Left leg is seen in abducted, externally rotated & semiflexed position. There is some wound above knee joint. If movements r restricted along with pain nd fever Two possiblities 1) Acute pyogenic arthritis of left hip 2) Acute reactive inguinal lympahdinitis Plz admit pt Adv CBC, CRP, ESR if possible Procalcitonin X-ray & USG lt hip Keep on IVF, IV Ceftriaxone Anti inflammatory Serraciopetidase Orthopedic opinion

Lt. Leg swollen, abducted, semi flexed at hip jt. A wound is present. D)D cellulitis, Septic arthritis, osteomyelitis, Investigation CBC Bl. Culture, X-ray, ESR, T/T I/v ceftrioxone, anti-inflammatory, serratiopeptides. Orthopedic opinion.

It's a spreading cellulitis in subcutaneous plane from the external wound with impending abscess formation. Start with antistaphylococcal antibiotics. I/D when abscess formation is complte and localised

It is likely to be cellulitis One has to watch for fasciitis Clindamycin should be included in regimen

Antibiotics ,AGRESSIVE treatment Prognosis guarded... Find out underlying cause....ie.anaemia, diabetes, perianal pathology, Organism .... streptococcus like ly...

Dx : RIGHT INGUINAL LYMPHADENOPATHY WITH LEFT THIGH INFECTION. (BACTERIAL INFECTION) D/D : T.B LYMPHADENOPATHY.
