A Ca Stomach patient with Large Bed Sores

A male aged 65 years who is a k/c/o Ca Stomach on chemotherapy Chief Complaints Developed sepsis with high grade fever, breathlessness and AKI Physical Examination Large bed sores as shown in picture Management Lets discuss the management options for this patient

Sepsis , high grade fever , breathlessness and AKI are part of septicaemia due to secondary infection in bed sore in an immuno compromised patient. Management 1.Use od air mattress 2.Frequent change of posture 3.parenteral antibiotics- piperacillon 4gm + tazobactum 500 mg B.D will be safe in AKI by reducing conventional 6 × hourly dose to B.D dose 4.AKI - requires nephrologist consultation and may require dialysis 5.Local debridement of the wound in O.T setting followed by dressings from a professional- preferably cleaning with Betadine lotion and dressings with sofratulle or megaheal gel 6.Once patient is out of AKI- protein and Zinc supplements, can be added
