Modified Hinchey classification of diverticular perforation and it's management 0- Mild clinical diverticulitis Treatment - Antibiotic Ia - Confined pericolic inflammation or phlegmon - Treatment Antibiotic Ib - Confined pericolic abscess - Treatment Antibiotic with pigtail catheter insertion to drain abscess II - Pelvic, distant intra-abdominal or retroperitoneal abscess Treatment - Antibiotic, pigtail catheter insertion and if required surgery III - Generalized purulent peritonitis - Treatment open exploratory laparotomy IV - Generalized fecal peritonitis - Treatment open exploratory laparotomy

Age THANKS DR Santish kumar Choudki for presentation of this interesting case in Curofy Experts clinical case. and also managed and treated with exoertise But as Surgery is the last resort in such cases ., we should look at the pre- disposing factors and causes which are not discernible AGE - As you get older, the walls of your large intestine become weaker and the pressure of hard stools passing through your intestines can cause diverticula to form. The majority of people will have some diverticula by the time they're 80 years old. DIET & LIFESTYLE Not eating enough fibre is thought to be linked to developing diverticular disease and diverticulitis. Fibre helps to make your stools softer and larger so they put less pressure on the walls of your intestines. Some other things that seem to increase your risk include: SMOKING BEING OVERWEIGHT OR OBESE HAVING A HISTORY OF CONSTIPATION long-term regular use of painkillers such as ibuprofen or aspirin GENETICS You're more likely to develop diverticula if you have a close relative with diverticular disease, especially if they developed it before they were 50.

Congrats,Dr Santosh Kumar Sir. Good job done. I have few observations to make with a request to take in their true perspective. Firstly about the title " CURE....." The condition is liable to recurr unless the predisposing factors are controlled post surgically. A high-fat, low-fiber diet is the main culprit in diverticulosis, or the formation and periodic inflammation of out-pouchings in the intestinal wall. Genetics and low physical activity levels may also play a role. So high fiber low fat diet should be advised as well as changing the sedentary habits. Secondly about the DD : None of the DD mentioned cause lower left quardent pain . Just the ritual of enlisting DD at the best. Post operative advise is as important as surgery from the point of view of CURE . REGARDS SIR.
