A case Rheumatic valvular heat disease with Mitral stenosis with cardiomegaly and pedal edema with 130 mgHg severe pulmonary hypertension. Mid diastolic murmur and early systolic murmur Medical treatment Betablocker, digoxin, sildenafil along with diuretics which decongested the patient and controlled the ventricular rate and pulmonary hypertension improved respiration and decongested the liver Broad spectrum antibiotics All this there was major improvement Further surgical management with open heat surgery using heart lung bypass. Mitral valve replacement with mechanical mitral valve Small clot in left atrial appendage removed and tricuspid was clasping well with annulus arround 38 mm Left atrial apindages lighted externally lt and Rt atrium closed externally Dr Rakesh chaudary I congratulate you for you nice and successful result and management

ASO titre & Throat swab C/S test to detect streptococcal infection routinely shud be done in suspected rheumatic fever and throat infection in children to prevent Mitral stenosis in time. Nice presentation on Mitral Stenosis following rheumatic fever.
