Case of the day

A female who underwent Lap Cholecystectomy for GB stones 2 years back p/w complaints of Pain Abdomen and Jaundice (progressive) since almost a month.... She also has pruritus and clay coloured stools... comment on her MRCP and discuss the findings

The liver releases bile salts into the stool, giving it a normal brown color. may have clay-colored stools if you have a liver infection that reduces bile production, or if the flow of bile out of the liver is blocked. Yellow skin (jaundice) often occurs with clay-colored stools Alcoholic liver disease Gall stone Hepatitis Hepatitis A Gastrointestinal disease MRCP shows that there is obstruction at small intestine Put rt n give enema Nbm till further order Surgeon opinion

Can be case of carcinoma or obstructive jaundice

Obstructive jaundice as there is dilated CBD and intrahepatic biliary radicles Most likely 1.Periampullary carcinoma 2.Cholangiocarcinoma, 3.Stricture lower end of CBD 4.Impacted stone lower end of CBD . Pain abdomen and jaundice is indicative of ascending cholangitis . Pruritus and clay colored stools indicative of obstructive jaundice.
