18 yrs old male pt having these types of nails of left hand since 2 month. no itching no other complaint plz dx n treatment

72 year old lady with cognitive decline and anxiety. Reports that her daughter stepped on her foot 5 weeks prior and the nail was lifted and bruised. There was a sweet/ fermented smell coming from the nail so I removed the nail plate to investigate. We have not taken cultures but are treating based on clinical signs. What bacteria is this and what treatment methods would you use?
