71 year old male taken by his family because of episodic altered mental status, agitation, and hallucinations since 1 month. The patient reports having insomnia, nonthreatening visual hallucinations and rarely auditory hallucinations. His medical history includes stage 2 hypertension,, ischemic cardiomyopathy, coronary artery bypass surgery, and dyslipidemia. A CT scan of the brain is attached, What are possibel diagnosis and management plans for this.

70 years old gentleman known case of diabetic and hypertensive on along a and metformin(1000mg) and glimipride( 4mg)...has a hemiparesis attack .. mri brian showed non haemorrhagic acute infarct involving medulla oblongata on right side... and generalised cerebral atrophy with bilateral small vessel chronic ischaemic changes.. blood reports are attached....kindly tell how to proceed further and what treatment I should put ....
