DLBCL - diffuse large B- cell lymphoma involving thyroid gland

67 years old female with a thyroid mass and loss of weight and appetite. USG - neck - Right thyroid gland mass lesion with level 2 cervical lymphadenopathy A PET scan shows - metabolically active large thyroid mass with metabolically active bilateral lung nodules . A core needle biopsy- A DIFFUSE LARGE B - CELL LYMPHOMA . Conclusion- A stage 4 - B- cell lymphoma Management plan in this case welcomed

Sir It’s DLBCL with thyroid involvement The Pt will need chemotherapy as this condition is chemo sensitive But it also has a relatively high recurrence rate So kindly refer to a Med oncologist

B cell lymphoma presenting as thyroid swelling is rarity Very interesting case We used to give R - CHOP regime for B cell lymphoma But one thing which I can assure you - field of medical oncology is evolving very rapidly Every other day a new molecule is introduced and approved by FDA New types of monoclonal antibodies have created a storm - they have totally changed the way cancer is treated with better and better outcome Chronic myeloid leukaemia, gastrointestinal stromal tumour were considered incurable at one point of time - but situation has dramatically changed after Imatinib mesylate and sunitinib - now we are looking at long term survival Medical oncology is one field where ordinary people like me can not keep pace with It is rapidly advancing field with better and better outcome for patients - best to be left to handful of medical oncologist!

Diffuse large B-cell lymphoma (DLBCL) tends to grow quickly. Most often, the treatment is chemotherapy (chemo), usually with a regimen of 4 drugs known as CHOP (cyclophosphamide, doxorubicin, vincristine, and prednisone), plus the monoclonal antibody rituximab (Rituxan)

Sir definitely it is DLBCL - diffuse large B- Cell lymphoma. Case has been discussed with medical oncologist and is being referred to him for chemotherapy. THANKS FOR THE VALUABLE OPINION

CHOP regimen, if CD 20 positive R-CHOP. And there are regimens like RICE.Pls refer to a medical oncologist as it is best handled by them.
