Abdominal pain in 7 years old
Male patient if age 7 years comes in opd with compalin of abdominal pain right side and near umblical since one month. Chief Complaints Abdominal pain Loss of appetite. History History of taking medication . Mefenamic acid. Metrogyl 200 mg tds. Neeri. Pan40. since one month but no improvement. Vitals Normal. Physical Examination Weak. Weight 35 kg Investigations Usg: mesentric lymphnodes are seen enlarge 11×5mm. Concretion in left kidney. Diagnosis Needto be discuss. Management Need to be discuss.
its mesenteric adenitis . give tab lcin oz 1 tab twice daily for 7 days . tab drotin m 1 tab thrice daily for 7 days . tab pan 40 1 tab once daily . cap vitazyme 1 cap twice daily for 15 days . do take history of contact with tuberculosis tooo .
Mesentric lymphadenitis size 11mm, not much significant for Koch's. Do CBC, esr, Chest Xray, CRP factor Give IV monocef 1gm bd Amikacin 125mg BD Meftsl spas bd All for 5 day's
Blood counts Urine ,stools exam Xray chest CT /US guided FNAC of mesenteric lymph node Probably a non specific mesenteric adinitis. DD TB adinitis Deworming can be done . Treatment after results are available.
Send Haemogram CRP Blood culture. Start Triple antibiotic regimen( IV cefotaxim ,IV amikacin ,IV metronidazole). Start analgesics (IV paracetamol, Tramadol ,meftal spas round the clock) . Pantoprazole (orally/ IV ).
? BOWEL PATHOLOGY.. ? MESENTERIC LYMPHADENITIS.. NEED'S.. ROUTINE INVESTIGATIONS.. CRP.. USG..ABDOMEN..CTCE..STUDY.. CXR HPE..STUDY..
Mesenteric Adenitis. To be treated with a combination of antibiotics and anti inflammatory drugs. 1. Syp Oflox 100mg/5ml - 10ml bd for 7 days. 2. Syp Ibugesic Plus - 7.5 ml tds for 7 days for inflammation and pain. 3. Tab Bandy plus (albendazole+ivermectin) - stat.Repeat after 2 weeks. 4. Cap Nilstone - 2 cap bbf for 9 days. Plenty of fluids. Investigations: ESR, Mx, Urine routine, Widal.

11 year male with complaints of pain abdomen. There was no H/O persistent fever/weight loss/loss of appetitie. There is H/O beef/pork intake in family. Blood reports were normal. USG abdomen showed pre and para aortic lymph nodes. Stool sample showed giardia which was treated with appropriate antibiotics. He recovered well but patient continued to have mild pain occasionally. MRI Abdomen was done. Mesenteric lymphnodes were still present. Is anything to worry..? And further workup required?

in daily clinical practice , i found lot of opd complaining for abdomen pain.mostly on P/A examination no any systemic abnormalities noted, in such cases after USG 90%patient s showed Mesenteric Lymphadenopathy what are the typical management of such patient, how much major role of antibiotics in this cases???
