Looks like rheumatoid arthritis with Swan neck n bouttannier deformity developing. Go for contrast bath, mobilisations, stretching n corrective splints. Also test for carpal tunnel syndrome for tingling n pain at night. Do NCV for the same.

Probably rheumatoid arthritis. But the neurological symptoms may suggest Carpal Tunnel Syndrome. Consider NCS. Delay in treatment can lead to permanent disability.

Though there is the appearance of developing RA related deformities, there is no visible synovitis. A bilateral CTS is less likely than simply Vitamin B12 deficiency, which must be considered due to the hyperpigmented knuckles and sensory symptoms. A vit. B12 test along with rheumatological history due to pain and possible deformity should help the diagnosis.

due to night exacerbation it could be, RA with Carpal Tunnels Syndrome or Cervical Spondylitis. But, Filarial Lymphangitis could be a primary or remote diagnosis. Filarial affliction is diagnosed by a slightly raised relative rise in eosinophiic blood count. More confirmativve would be a positive night blood thick smear for micro-Filarial parasite. The treatment is simple, Tab. Diethylcarbamazine (Tab. Hetrazan or Banocide), 100 mg TDS, for 21 days. This should settle the dispute and givve long lasting relief and cure. Good Luck Dear.
