Ankle arthrodesis when talus is lost following open ankle dislocation

45 years male wraith history of fall from hill top 3 hrs back following which sustained injury to rt ankle from the site of fall he ran up to 300 to 400 mts due to fear of bear then he realized bleeding ankle and difficulty in walking O/e Eposed dislocated ankle with missing talus and contaminated mud and Pollen straws with palpable pulsation and toe movements are present Now only management is dedridemnt and arthrodesis r any other suggestions ? Clinical picture and x ray been tagged

Xray show calcaneum spur and rest normal Check the movement of the ankle joint and strength and cartilage Clean and wash all the foreign body from wound before stitching If possible before stitching debridement want then do accordingly Repeat cray after stitches and Oral medicine accordingly After stitches apply temporary plaster for 2 to 3 week

Debride thoroughly Apply external fixator connecting tibia and calcaneum Let the wound heal After that apply charnley's compression device and bone grafting to achieve tibio calcaneal arthrodesis

Arthrodesis is the only option, but most challenging task is without talus bone Limb length has to be compromised
