Dr Waseem - as you belong to Srinagar, Ascariasis in kashmir Valley is very common Many unusual presentations of Ascariasis have been reported in research papers in kashmir like 1.Chest symptoms like wheeze , crepitations during pulmonary migration 2.Acute Appendicitis 3.Acute cholangitis 4 Acute Cholecystitis and biliary colic 5 Acute Pancreatitis 6.Hepatic abscess 7. Bolus intestinal obstruction 8.Volvulus 9 Neurological symptoms- particularly in bolus Obstruction when antihelmenthic has been given and neurotoxic enzyme released from dead worms 9.Nutritional deficiencies. 10 Acute gastritis with vomiting of worms or expulsion from nasal passage as in this case. As a general surgeon- I too have operated a case of CBD obstruction due to ascariasis

Pulled this worm out And deworming continuous for 3days there after repeat the dose after 3 weeks keep followup Treat nutritional supplements Adv and edjucate about hygiene Rt lumbar pain need to subject fir 7sg abd and kub D/d appendicitis Renal calculus or uti

Piprazine citrate ( 75 mg/kg/day for 2 days , maximum dose:3.5 gm),as syrup through NG tube, treatment of choice for intestinal or biliary obstruction. Other treatment option- Albendazole ( 400 mg orally) , mebendazole ( 100 mg orally BD for 3 days) . ivermectin ( 150-200 ug /kg , OD).
