Rural diabetic anaesthetic foot.

He is Mr X ,from the remote rural India ,who is illiterate and ,deaf and socioeconomically backward ,was reported to me.His diabetic status was managed by oral antidiabetic drugs .Daily dressing with ,H2O2 cleaning ,saline irrigations ,and iodine solution soaked dressing with cloxacillin oral use gave fast recovery to this poor man. His attention towards the wound and medical care was not much appreciative .Inspite of that draw back,his wound was wonderfully healed ,because of his good tissue healing power by granulation. Every day I dress the wound with high case but he was walking down to my clinic on next day with barefooted legs ,and full of filth and mud. To my surprise his wound have obeyed to the clinical management , wonderfully was really unbelievable. This scenario is visible if any one examine his feet carefully how unhygienic it is . That is Indian rural foot. In a matter of 10 days his diabetic foot ulcer healed well. Experience vary from patient to patient. No Xray ,No CBC,Hb,No Doppler, Capillary blood glucose random manner was monitored on every 5th day. No Insulin . Total anesthesia of feet was.detected clinically. During Corona clamp down he is regularly coming for dressing by barefooted walk,due to restrictions of vehicular movements.

This is clear cut case of callous of plantar / sole ,that gave way for invasion of bacteria in an rural diabetic foot who is barefooted. Lucky enough that abscesses is firmed on the dorsum of foot and it was drained and wound healed. Diabetic barefooted clawing present Anaesthetic feet. badly maintained poor hygiene so infection invasion on oral antidiabetic drugs. but his foot responded to treatment Inspite of bad hygiene is due to good immunity. He is alcoholic. Individualization of diabetes is more approximately chosen in managing the diabetes. .

Pyoderma gangrenosum .. It is a rare, inflammatory ...autoimmune skin disease where painful pustules or nodules become ulcers that progressively grow.  Pyoderma gangrenosum is not infectious. Treatments may include corticosteroids, ciclosporin, infliximab, or canakinumab. Second probability is multiple corns due to compression on bare foot.
Dibetic Foot leech therapy should be done,wash wound with trifla and neem kashay,smriti sgar rasa,rajat bhasma,praval pishti,bramhi churna,jamun guthli churna and gudmar patti churna kwath,panchgavya ghrita..,amritarishta,and maintaing og b. sugar by appropriate aahar vihar is most imp.
