Ice fomentation Limb elevation Mgso4 dressing If thrombophlebitis, consider thrombophobe.
Thrombophlabitis. Multiple puncture at site of inj. Must be avoided.and extravasation of iv fluid or counter puncture avoided so vain is not inflamed and not leads to thrombophalabitis. Rx-lioten or thrombophobe oint locally. Local massage of the part. Tab seradic plus 1×3 ×5days.
Obviously Thrombophob ointment locally Icepack fomentation Chymoral forte 1bd Symptomatic treatment
Icepacks Thrombophobe oint locally Or Hirudoud oint Sos NSAIDs
What I advise is - Chymoral forte TDS, Signoflam BD, Oint Thrombophob L/A thrice daily after Ice application
KIND OF DRY HEAT FOMENTATION ALLWAYS KEEP THE CANULA PATENT APPLY THROMBHUPHOB OINT AT TGE SITE OF ERYTHEMA /SWELLING

76yr old female diagnosed Interstitial lung disease.... Presented with fever breathlessness to casualty After careful examination Culture showed kleibshella ...pneumonia... Later patient was put on antibiotic according to culture report..... No improvent seen Patient still had wheeze and breathlessness and fever Changed the antibiotic to meropenam and colistin Still no improvement is seen Today ct showed image 1 Cavitation???? Fungal ball??? Aspergillosis?? But previous ct and X-rays in 2016 showed cavity too.... Patient is kept on ntg as Bp is 180/100 in titration dose 3ltrs of oxygen.... without tat patient not able to maintain saturation Ecg showed some irregular rhythm was taken advice from cardiologist and put on calaptin tablet 40.... Please suggest further suggestion n opinion

Pt is 70 year old and farmer by occupation comes at health center with folowing complain. 1)ulcer on posterior on rt great toe. 2)ulcer on posterior little finger. past history pt was treated fo mb leprosy with mdt 2 years back.there is no sensory loss except patches.please advise the treatment and follow up.
