C-peptide estimation Fasting or post prandial which is best?
C peptide level are indirect measure insulin production in our body Low C peptide levels may confirm absence of inherent insulin secretion and this justify use of Insulin for treatment. In fasting state, few people may have hypoglycemia, which will inhibit native insulin secretion, thus low C Peptide level may not be representative of absence of insulin secretion Post lunch there is hyperglycaemia, our body is forced to secrete insulin for correcting hyperglycaemia Therefore if C peptide levels are very low in post meal period it may represent true absence of secretion of insulin and true insulin deficiency Thus post meal C peptide levels are more specific for finding out absence in native insulin secretion
Correct method is to estimate the C-Peptide level during the morning hours especially when the person is in the overnight fasting state (8-10hrs post dinner/last fed).
OVER NIGHT FASTING IS GOOD.. AS PLASMA GLUCOSE LEVEL IS RELATIVELY STABLE ..DURING FASTING.. & INSULIN SECRETION IS ASSUMED TO BE STABLE..
Always done in fasting stage for correct estimation
Value on fasting sample of 10 hours is more reliable.
8 hrs overnight fasting is ideal
Over night fasting is good way to collect

56 years old female with 18 history of diabetes and high blood pressure in management with insulin and losartan with no major cardiovascular events at this time. She's been complaining of this problem one week ago, this lesions started in the groins and now there are in her armpit, arms nach chest. This lesions start like the first picture and suddenly take the appareance of the second one, are painful. The whole skin examination shows nothing more. What's your suspicions?
