HYPERTENSIVE BLEED

60yrs old Hypertensive male presented with loss of consciousness this was followed by headache and dizziness. O/e - Unconscious,gasping, Decerebrating,Pupils - B/l 7mm NRTL,GCS - E1M2V1,BP - 200/100mmhg,PR -42/admitted under Neurosurgeon and explained poor prognosis to relatives.Relatives opted for conservative management only.

A massive hypertensive berry aneurysmal rupture in the Pontine, left para ventricular region with left lateral intraventricular extension . The nature of bleed carries a poor prognosis due to severe global cerebral ischemia,and obstruction in the outflow of CSF .. Although neurosurgical intervention may save the patient but with high mortality outcomes . Meanwhile conservative treatment with gradual lowering of B.P with labetalol drip can be started
Lt thalamocaosuloganglionic bleed with peroration to lateral ventricles 3rd and4th ventricle with early hydrocephalus. Suggest supportive management.
