A young patient of 18 yrs admitted with c/ o fever for past 30 days. No other symptoms. On examination nothing significant found except fever. All investigations were normal . CXR and CT chest is posted here for your valuable opinion.

Good morning Dear Friends, Presenting a case of a boy , 10yr old who was having history of fever, headache n vomiting for last 10 days. Had history of exertional dyspnoea. His father gives history of him becoming bluish when he used to cry at age of 4 to 5 years old. But due to financial constraints never got any medical help. Present Examination revealed Grade 3 Clubbing, Cyanotic nail bed, suffused conjunctiva, systolic thrill is present anteriorly along the left sternal border. A harsh systolic ejection murmur (SEM) is heard over the pulmonic area and left sternal border. The S2 is single. His Blood investigation revealed Hb 18.5 & HCT 63. WBC was 23000 with shift to left. RFT, LFT were normal. X ray is placed below. CECT Brain Revealed an Abscess. Q.. 1. Diagnose the clinical scenario. 2. Detailed management plan for this young boy. Regards.

A young female aged 18 yrs presented with history of Fever with chills , breathlessness since 5-6 days and altered sensorium since 1 day.....On taking history from attendants, it was revealed that She has had 2-3 episodes of Jaundice in last 2 yrs and 3 PRBC transfusions were also done 8 months back due to severe anemia....She was foundto have clubbing...Discuss this interesting case and possible differentials...

### 45 years old Muslim housewife got admitted with the complaints of joint pain for 2 & 1/2 years. Associated with generalised purpuric & non blanching rash for 3 days which is more on both lower limbs & upper limbs. H/o mild fever for last 3 days associated with oral ulcer.
