CHRONIC SUBDURAL HEMATOMA

68yrs old female admitted with Unilateral weakness and vertigo,Her son reported that she 20days before,she had accidentally fallen and hit her head and had been admitted to hospital,At that visit she had no neurological symptoms except for headache and no motor weakness.Not known comorbid,Vitals unremarkable.DIAGNOSIS AND APPROACH?

Right subacute on chronic sdh with multi septations and membrane formation causing mass effect and midline shift. Pt needs craniotomy and hematoma evacuation with careful cauterization of the membrane edges which are source of bleed in post op period ofcourse after normalising his blood parameters

Acute on chronic SDH rt frontotemporoparietal with sepatations and compartmentalization .maximum thickness rt frontal ant temporal with uncal herniation to left midline shift ,effacement of rt lateral ventricle. Needs urgent Neurosurgical intervention
