What are these lesions?

87-year-old male with history of HTN, AV block with PM, treated with lercanidipine, irbesartan, furosemide, pantoprazole, presents to the ED with rest dyspnea. Came across this lesion while examining him. Any ideas? Can actinic keratosis be this large?
