A 75 year old man is admitted to ICU from cardiothoracic ward. He had uneventful coronary artery bypass surgery 2 days ago and had been stepped down to the ward yesterday. He is breathless, pale, clammy and light headed. His heart rate is 110bpm, with a blood pressure of 90mmHg systolic, and his peripheral saturations are 90% on 6L/min via Hudson mask.
