Chronic UTI

F/55, C/o. Low back pain and burning micturition. No other complaints. First Urine R/M shows numerous puss cells. USG shows Renal calculi with rt. Renal exophytic cyst, cystitis and was prescribed Cepodem 200mg twice daily for 5 days with no results. So urine culture and sensitivity was done which shows Escherichia coli. Based on sensitivity she was prescribed oral fosfomycin 3gm 3 doses 3 days apart because she was not ready for I.V. therapy with Piperacillin / Tazobactam. Presently after fosfomycin course all her symptoms improved and Urine R/M showing 100 to 120 puss cells. The question is that this remaining puss cells will be cleared soon or she needs further fosfomycin 3gm continuation therapy? Please advise with further course of management in such cases.

A case of chronic UTI needs some other,investigations also like 1 Ultrasonography abdomen for renal calculi and other congenital anomalies of Urinary tract which can pre- dispose to UTI 2 IVP 3 Cystoscopy to rule our bladder neck contracture Treatment- Give tab Cranpac D - containing cranberry extract and D- mannose once a day for 3 months. It has no side effects as it is plant extract and quite effective in chronic UTI

First get urine to be investigated for C.& S. USG Abdomen/Pelvic Some times Nitrofurantoin 100 mg 1 tab OD x 3 months need to be continued with Alpha Blocker..

Chr UTI Needs further investigation and evaluation to conclude particularly USG Abdo urine C and S any ivp cystoscopy. Symptomatic treatment till reports complied particularly BS antibiotics iv bd slowly and analgesic SOS. Plenty of liquid daily. Avoid using oily spicy and fast food.

advise him to take plenty of water vrunadikwath sarvatobhadra vati gokshuradi guggle

Needs to continue antibiotics but I also think repeating CUE again may be worthwhile. Just in case the report is wrong
