Copper T arms missing

Hi needed valuable advice from seniors Wat happens if the arms of copper T is left behind in the uterus and insertion the new one in the same setting ???

- how can arm left behind in Uterus ? - why not seen removed part of CuT that give something missing ? - and when did that error came to notice ? if it noticed immediately then can be rectified at same time. - as such nothing happens to patient but it will give opinion of ecxellancy of that gynaecologist, when patient may go others and observed about the error encountered by previous one.

If copper T removed with misplaced arm, Adv USG to locate missing arm. D/C and diagnostic hysteroscopy would be advisable.
